Beneficiary-submitted claim filing instructions

First Coast processes claims for services rendered in Florida, U.S. Virgin Islands, or Puerto Rico. If a beneficiary needs to submit a claim form, the beneficiary should complete and mail the Patient Request for Medical Payment form (CMS-1490S) to:

You should only need to file a claim in very rare cases

Medicare claims must be filed no later than 12 months (or one full calendar year) after the date when the services were provided. If a claim has not been filed within this time limit, Medicare can't pay its share. For example, if you saw your doctor January 10, 2024, the Medicare claim for that visit must be filed no later than January 10, 2025. Please check the Medicare summary notices (MSN) you may have received in the mail or go to Checking the status of a claim to ensure your claims have been filed in a timely manner. If your health care provider has not filed the claim, please take one of the following steps:

  • Contact your doctor or supplier, and ask them to file a claim.
  • Please call 1-800-MEDICARE and ask for the exact deadline for filing a Medicare claim for the services or items you received. If you discover that the deadline is imminent, please complete and mail the CMS-1490S form.
