Do not report more than one unit per date of service for E/M codes
First Coast would like to remind providers when billing E/M codes within the CPT code range 99202-99239, do not report more than one unit per date of service. Billing more than one unit for these services on the same date of service will cause your claim to reject with group code CO16 and message code N345, indicating the date range is not valid with the units submitted.
If you receive this message on your remittance advice, you will need to make corrections to the units billed and resubmit your claim.
