Medicare Coverage Database code and keyword search

To search the Medicare Coverage Database (MCD) for First Coast's LCDs and related articles, enter your keyword(s), code, or document id below.

Note: Not all codes have a policy associated with it. First Coast will consider reasonable and necessary services for payment in the absence of an LCD, billing and coding article, NCD, or CMS manual instruction that limits coverage.

Note: Looking for CPT and HCPCS Code Tables or a related covered diagnosis? Per CMS CR-10901, codes were relocated from the LCDs into their corresponding articles. When searching by code, if a policy does not return, be sure to view related articles listed in the search results, as they may reference any associated LCDs.

MCD search tips

LCD #: Searching by the LCD #, when available, is the recommended search method. Simply enter the LCD #, for example "L33910", and click Search. 

CPT/HCPCS code search: No LCD #? Try a procedure code, such as: "78815". For best results, avoid extra keywords such as "CPT 78815" or "78815 Pet scan".

ICD-10 diagnosis code search: You may also search with a diagnosis code, such as "E08.00". For best results, avoid extra keywords such as "ICD -10 E08.00" or "E08.00 Diabetes mellitus". Note: ICD-10 codes that are included inside a range of codes will not pull up in the search. 

Multiple keywords: Search with descriptive terms, such as: "hyperbaric oxygen therapy". Be sure to check your spelling. 

Preventive service codes: Please view the CMS interactive Preventive Services chart  for coverage guidelines. The chart includes HCPCS / CPT codes, diagnosis codes, coverage requirements, frequency requirements, and beneficiary liability. 

Older dates of service: If you are looking for older dates of service and if the results page indicates no records returned for articles and/or LCDs, try using CMS' MCD Archive.
