Section I: Active Diagnosis
I0020 Indicate the resident's primary medical condition category
- Report the diagnosis that is cause for the skilled need
- Supportive documentation from the three-day qualifying stay, SNF admission notes, physician's admission history and physical (H&P), therapy evaluations and notes
I0020.I Active Diagnoses in the last seven days - Check all that apply (I0100 -I8000)
- Supportive documentation from the 3-day qualifying stay, SNF admission notes, physician's admission H&P, therapy evaluations and notes, physician's orders
- Nutritional- I5600 Malnutrition (protein or calorie) or at risk for malnutrition:
- This is a common error and cause for recoding the PDPM. Supportive documentation must be from the physician: admission H&P, physician's orders, physician signing and agreeing to the dietician’s assessment.

J1100c Shortness of Breath (dyspnea)
- Occurs within the seven-day look back period
- Supportive documentation from nurses/physicians notes, respiratory notes, therapy notes, physicians orders, ADLs
J1550 Problem Conditions (within seven-day look back period)
- Supportive documentation from nurses/physicians notes, therapy notes, physicians orders, ADLs, vital signs, 3-day qualifying stay record that includes surgery notes
